Revive Policies

Revive Med Aesthetics Policies

At Revive Med Aesthetics, we value your time and are committed to providing exceptional care in a safe, welcoming environment. The following policies help us maintain appointment availability and ensure the best experience for every patient.

Appointments

A valid credit or debit card is required to reserve all appointments.

Certain appointments may require a non-refundable deposit. Your deposit will be applied toward your treatment at the time of your visit.

Appointment reminders are sent as a courtesy. Failure to receive a reminder does not relieve patients of the responsibility to keep their scheduled appointment.

Cancellation & No-Show Policy

We kindly ask for at least 24 hours’ notice to cancel or reschedule your appointment.

Appointments cancelled or rescheduled with less than 24 hours’ notice will incur a $100 cancellation fee.

Patients who miss their appointment without notice will incur a $200 no-show fee.

Cancellation and no-show fees will be charged to the card on file. If payment cannot be processed, any deposit on file will be applied toward the outstanding balance. Any remaining balance must be paid before future appointments may be scheduled.

For prepaid treatment packages, Revive Med Aesthetics reserves the right to charge the applicable fee or deduct the missed appointment from the treatment package.

Deposits

Deposits are non-refundable.

If you cancel or reschedule at least 24 hours before your appointment, your deposit will remain as a credit on your account and may be applied toward a future appointment or eligible service.

Treatment Packages

Treatment packages are non-transferable and must be redeemed within one (1) year of the original purchase date unless otherwise specified.

Packages may be refunded within ten (10) days of purchase provided no treatment has been redeemed. Once any treatment within a package has been performed, the package becomes non-refundable.

Memberships

Memberships require a minimum six (6) month commitment unless otherwise stated.

Members may cancel after the initial six (6) month commitment by providing at least 30 days’ written notice.

If a membership is cancelled before the initial commitment is fulfilled, an early termination fee equal to one (1) month’s membership payment will be charged.

Membership benefits are available only while the membership is active and in good standing. If a scheduled membership payment cannot be processed, membership benefits, including member pricing, discounts, and exclusive offers, will be suspended until the account is brought current. Any available monetary account balance will remain on the member’s account.

Refunds

Due to the nature of aesthetic medicine, results vary from person to person. Refunds are not provided for services rendered or dissatisfaction with treatment outcomes.

Retail skincare products may be returned within thirty (30) days if unopened, unused, and in their original packaging. Opened or used products are not eligible for return unless determined to be defective.

Gift cards are final sale, non-refundable, non-transferable, and may not be redeemed for cash.

Promotions & Financing

Promotional offers cannot be combined unless otherwise stated and cannot be applied to previous purchases.

Revive Med Aesthetics proudly accepts Cherry and CareCredit financing for qualifying purchases.

Children, Guests & Pets

For the safety of our patients and to ensure the best treatment experience, we ask that children, additional guests, and pets not accompany you to your appointment unless they are receiving services or are a certified service animal. Children may not be left unattended in the reception area or treatment rooms.

Health Changes

Please notify us before your appointment if you become pregnant, start new medications, experience significant changes in your health, or develop an active illness or infection, as certain treatments may need to be postponed for your safety.

Right to Refuse Service

Revive Med Aesthetics reserves the right to refuse or discontinue treatment for patients who repeatedly violate office policies, maintain unpaid balances, file unwarranted chargebacks, display abusive or inappropriate behavior, or fail to follow treatment recommendations.

Acceptance of Policies

By scheduling an appointment, purchasing products or services, or receiving treatment at Revive Med Aesthetics, you acknowledge that you have read, understood, and agree to these office policies.
